9/27/2023 EnchantaVaughn Approved QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival to the home setting, Ms. Broadnax appeared to be in a relaxed mood evident by her facial expressions such as a smile on the face and engaging in appropriate conversation with QMHPA. QMHPA inquired about her day. Ms. Broadnax did not present any symptoms of S/I or H/I at the time of the session. QMHPA began the session by inquiring about Ms. Broadnax’s current thoughts, feelings and emotions. QMHPA educated Ms. Broadnax on stressors and how they can impact daily functioning skills. QMHPA reviewed with Ms. Broadnax appropriate and inappropriate ways to manage stressors. QMHPA engaged Ms. Broadnax in a skills building activity. QMHPA explained to Ms. Broadnax that the purpose of this activity was to assist. Ms. Broadnax with effective communication skills to express her thoughts when she is experiencing depression/anxiety. QMHPA explained to Ms. Broadnax the benefits of displaying as well as promoting positive self-awareness at home and throughout the community. QMHPA developed a chart with Ms. Broadnax that will display her mood throughout the week. QMHPA explained to Ms. Broadnax she is to create a smiley face when she is feeling good about herself and sad face when she is experiencing depression or sadness. QMHPA processed with Ms. Broadnax trusting others throughout the community and her family members. QMHPA explained to Ms. Broadnax developing trust in others and letting go of negative thought patterns she has about others. Ms. Broadnax appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Ms. Broadnax replied "ok" when QMHPA discussed daily stressors. Ms. Broadnax said she walk around to handle her stressors. Ms. Broadnax said she feels sad sometimes when she is alone, but at the same time she wants those people around that get n her “nerves”. She indicated to QMHPA that she doesn’t want her daughter to be in her presence but when it comes to her boyfriend that is a different story because he at least bring her some type of laughter. Ms. Broadnax was excited about creating a chart with faces expressing her positive or negative self-esteem. Ms. Broadnax placed a face without a smile and said she is not sad nor happy it’s just a normal look. Ms. Broadnax discussed with QMHPA that she doesn’t trust to many people just from the childhood that she had when she was growing up.
